CPAP & BiPAP therapy in Coimbatore — titration, mask fitting and follow-up

A sleep study tells us whether you have obstructive sleep apnoea and how severe it is. CPAP or BiPAP therapy is what actually treats it — and the difference between therapy that works and therapy that ends up in a cupboard is almost entirely in the setup and follow-up.

At Lung Care we titrate the pressure to your own airway, fit and re-fit the mask until it is comfortable, set humidification for the Coimbatore climate, and then review your machine's usage and residual-event data at each visit. BiPAP is used where CPAP is not enough — obesity hypoventilation, COPD with carbon-dioxide retention, overlap syndrome and selected neuromuscular disease.

Who is this for?

  • Moderate to severe obstructive sleep apnoea confirmed on a sleep study
  • Mild sleep apnoea with significant daytime sleepiness or heart disease
  • Obesity hypoventilation syndrome or daytime carbon-dioxide retention
  • COPD-OSA overlap syndrome, or COPD with nocturnal hypoventilation
  • Patients already on CPAP who find it uncomfortable or are not improving

What happens during your visit

  1. We review your sleep study, oxygen levels and any heart or lung disease before choosing CPAP, auto-CPAP or BiPAP.
  2. Pressure is titrated — attended overnight in the lab, or by auto-titration data at home.
  3. Mask type and size are fitted, and leak, humidity and ramp settings adjusted for comfort.
  4. We review machine data (usage hours, residual AHI, leak) at 1 month, 3 months and then annually.

Why this test is performed

  • To hold the upper airway open through the night so breathing pauses, oxygen dips and arousals stop.
  • To restore refreshing sleep and reduce daytime sleepiness, morning headache and concentration problems.
  • To support blood-pressure control and reduce the cardiovascular load of untreated sleep apnoea.
  • To provide ventilatory support where carbon dioxide is retained overnight, using BiPAP rather than CPAP.

How to prepare

  • Bring your sleep study report, and a list of all your medicines.
  • If you already own a machine, bring it along with the mask and the SD card or app data.
  • Tell us about nasal blockage, sinus surgery, reflux or claustrophobia — all of these change the mask choice.

What it feels like

  • Air is delivered through a soft mask; it feels like a steady breeze rather than being pushed to breathe.
  • Most people need a week or two to get used to it. Early nights may be broken; that is normal and improves.
  • Leak around the eyes, a dry mouth or nasal congestion are usually mask-fit or humidity problems we can correct.

Safety and limitations

  • Genuine complications are rare — nasal dryness, congestion, skin marks from the mask and mild bloating are the usual issues, and all are adjustable.
  • A machine bought online without titration and follow-up is the commonest reason therapy fails.

Results and reporting

  • We review usage hours, residual AHI and leak from the machine's own data at one month, three months and then annually.
  • Pressure, mask and humidification are adjusted at each review rather than left on the original setting.

When to consult a pulmonologist

  • You have been told you have sleep apnoea but never started treatment.
  • You use CPAP but still feel sleepy, or you use it for less than four hours a night.
  • Your mask hurts, leaks, or you wake with a dry mouth or headache.
  • You have COPD, heart failure or obesity with breathlessness and daytime drowsiness.
